Dentistry For Children: Serving Families in the GTA

Dear patients and families:

This is to notify you that after 37 years in this location, I am selling the paediatric dental practice located at
4190 Finch Ave East, Suite 407, to Dr. Deborah Lin and moving the active records and records that
were inactivated in 2021 and on to her practice at 4040 Finch Ave East, Suite 201. The practice is located at the
corner of Finch and Milliken, one block west of the current location, with ample parking. As of December 1,
2025, Dr. John Wiles, our hygienist, Bless Santaolaya, and I will continue working at Dr. Deborah Lin’s
paediatric dental practice. Dr. Iris Daniel has decided to step back, but her patients can continue treatment
with me, Dr. John Wiles, Dr. Deborah Lin, or Dr. Teresa Berger at the new location. We hope you will
continue with us in the new location, as we have truly enjoyed being part of your dental journeys, and we
hope you feel the same way.

Should you require a copy of your X-rays, charts or financial records prior to December 1, 2025, please feel
free to email or call our office at the above numbers. After December 1, 2025, the phone number and email
will still be available, but Dr. Deborah Lin will be the custodian.

If your file closed in our office before January 1, 2021, your paper chart and X-rays taken before November
2012 will be stored at RSRS medical storage facility. You can reach them at info@rsrs.com, call  1-888-563-
3732, or fax 1-877-398-5932. Digital photos and X-rays taken after November 2012 will be kept at Dr.
Deborah Lin’s office, where you can request a copy if needed. Any chart requests from RSRS, will have a
fee as per their standard protocols. These charts will stay in my possession until February 15, 2026 should
you require a copy, after which it will be at RSRS. I recommend contacting Dr. Lin’s office where they can
guide you to the information that you may require.​

 

All charts will be retained until the patient reaches age 28 or for 10 years after their last visit, whichever

period is longer.

 

I do this with mixed emotions as I have truly enjoyed helping my patients over the many years, and I feel the
transition to Dr. Lin’s office is a good fit and solution at this stage of my career. I thank you for your
continued confidence and support and I know that I have always given my all to make your child’s
experience as comfortable as possible while providing quality care. It has been a blessing and a pleasure
throughout the years.
